REQUIRED ENROLLMENT DOCUMENTS

State and federal law requires certain documents as part of the enrollment of students who are new to a school district.

These documents can be uploaded during the registration process. Most smart phones will scan or take a picture of your documents if you do not have a scanner.  

If you have any problems or questions in this regard, please contact the appropriate campus at the numbers listed below.

 

  • Birth Certificate

  • Proof of parent’s identity (Example: driver’s license)

  • Immunization Record

  • Social Security Card

  • Proof of residency (utility bill, i.e. water bill, gas bill, or electric bill). This is not required if the student is homeless.

  • Report card from prior school year, if available. Incoming 9th grade students are required to present proof of promotion.

LATCHKEY REGISTRATION

Download the registration packet to enroll your student into Latchkey.

The primary purpose of the SISD/SFY Latchkey Program is to provide children with a safe place to be until parents are able to pick them up. The program is an after school service offered at a nominal fee for the children of SISD. The program offers homework assistance (when able), enriching activities as well as a nutritious snack.

STUDENT INSURANCE

Sign up for coverage that provides financial assistance with your out-of-pocket medical expenses when your student sustains an accidental bodily injury.

Available for all students PK-12.

FAQs

  • If you already have a Skyward account and also have a new student you may enroll them through your current Skyward account and use the Only New to the District tab. 

  • Yes. While your child may remain enrolled at their campus, families must log in and complete returning student registration each year to update contact information, emergency contacts and required forms. Registration is not automatic, and enrollment is only finalized once this process is completed. 

  • If you forgot your login and/or need assistance with Skyward, click below and then click the Forgot Login/Password link under the login area or contact the campus for assistance.
