Parental Consent for Services (SB 12)
Authorized Clubs & Organizations
In accordance with law, the District must receive written parental consent before a student is permitted to participate in a District- or campus-authorized or sponsored club. This form must be signed and returned before the student will be allowed to participate in the club / organization.
Updated consent or revoking of consent for any individual club or organization can be submitted to the campus office or sponsor in writing at any time.
Students without appropriate parent consent for participation will not be allowed to participate in the club or organization referenced.

No, this law only applies to mental and behavioral health-related counseling services, not academic counseling.

Yes. Under the law, a child may consent to their own counseling in certain cases without a parent’s consent, including for suicide prevention, chemical addiction or dependency, or sexual, physical, or emotional abuse. School staff can also verbally inquire about a student's daily well-being without consent.

If you do not provide consent for routine health services, school staff will be unable to assist your child with minor illnesses or injuries, except in life-threatening circumstances. If you do not consent to mental health services, a school counselor or other staff member will still assist your child if they believe there is a risk of the student harming themselves or others.

Yes, you can give or take back your consent at any time by providing a written notice to your school's office.

SB 12 requires written parental consent for a student to participate in any district- or campus-sponsored club. For our district, this consent will be managed on a club-by-club basis. You will receive a separate permission slip from the club's sponsor for your child to participate.

This applies to all district- or campus-authorized or sponsored clubs, including academic clubs, interest-based groups, service organizations, and extracurricular activity clubs. The list of clubs offered at each SISD campus is available above on this page.

No. Students may not participate in or attend club meetings until a signed parental consent form is on file.
